Job summary

UnitedHealth Group seeks an Advanced Practice Clinician (NP/PA) to join the Senior Community Care (SCC) team. This on-site, part-time 20-hour position includes a $10,000 sign-on bonus for external candidates and benefits.

Qualifications

• Active certification for NP or PA and state licensure as applicable
• Unrestricted licensure with no disciplinary actions
• Access to reliable transportation and a valid driver’s license

Responsibilities

• Conduct comprehensive assessments including medical history, meds reconciliation, vitals and physical exam
• Support members during short-term sub-acute rehab
• Ensure accurate documentation including ICD-10
• Collaborate with interdisciplinary care team
• Conduct advanced illness and end-of-life discussions
• Provide counseling and education to patients and caregivers

Education

ANCC/AANP Family, Adult, Geria tric, or Adult-Gerontology Primary Care certification
