Job summary

Ennoble Care is seeking an experienced Nurse Practitioner or Physician Assistant to provide primary and palliative care to homebound patients in the Raleigh/Durham area on a part-time basis. You will work under physician supervision, conducting exams, formulating care plans, and coordinating with our clinical team to deliver high-quality home-based care. Preference for geriatric primary care experience; strong communication and independent problem-solving are essential.

Qualifications

• Board Certified NP or PA with North Carolina certification.
• Strong clinical skills with the ability to assess and diagnose.
• Independent practitioner with good problem-solving abilities.
• 1–2 years of primary care experience with geriatric patients preferred.

Responsibilities

• Provide primary and palliative care in and around Raleigh, NC.
• Perform exams, obtain histories, and create care plans.
• Communicate with clinical operations across platforms.
• Maintain patient-centered care for a panel of patients.
• Attend quarterly medical staff meetings.
• Develop impressions, diagnoses, and treatment plans.
• Consult with patients and families during visits.
• Order diagnostic testing as needed.
• Prescribe medications and necessary interventions.

Purpose:

We are looking for an experienced Nurse Practitioner to provide primary care to homebound and palliative care patients in the Raleigh, NC region on a part-time basis. You will apply medical techniques and principles under the supervision of a physician, and you will care for a diverse set of patients with chronic diseases.

What You'll Do:

• Provide high quality care to primary and palliative patients in and around (Raleigh, NC)
• Assess patient's health by performing physical examinations; obtaining, updating, and studying medical histories and creating the best plan for care.
• Effectively communicate with office staff and clinical operations across various internal platforms
• Provide high quality, patient centered care to a panel of patients
• Attend quarterly medical staff meetings
• Develop impressions, diagnosis, and treatment plans
• Foster trust and consult with patients, families, and caregivers during visits
• Order diagnostic testing as medically necessary
• Prescribe medications supplies and appropriate medically necessary interventions

What You'll Need:

• Board Certified, with a valid NP or PA certification in the state of North Carolina
• Strong clinical skills with the ability to accurately and confidently treat, assess, and diagnose
• Positive demeanor and strong people skills
• Ability to confidently problem solve and operate independently
• 1-2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required)

Salary Range:

• Fulle Time - $110,000 - $135,000+ (depending on experience and other incentives)
• Part-time providers are Paid Per Visit
