Job summary

Crossroads is seeking a part-time Nurse Practitioner to join our Suffolk, VA outpatient clinic. The role focuses on evaluating and managing patients with opioid use disorder, prescribing appropriate medications, and coordinating holistic care with the clinic team.

Ideal candidates hold an active NP license, DEA registration, and Medicaid/Medicare enrollment eligibility. Prior addiction medicine experience is preferred. Schedule includes two mornings per week with 5am–10am slots.

Qualifications

• Active, unrestricted NP licensure in the state of practice.
• Primary specialty in Family Medicine/Family Practice, Internal Medicine, General Practice, or Psychiatry.
• DEA Certificate of Registration in the state of practice with ability to schedule 2, 2N, 3, 4, & 5.
• Must have or be eligible for enrollment for Medicaid and Medicare credentials.
• Additional education or certification may be required depending on state of practice.

Responsibilities

• Evaluate and manage patients with opioid use disorder in outpatient clinic settings.
• Prescribe appropriate medications and treatments as indicated by patient assessment.
• Complete medical documentation per Crossroads standards.
• Collaborate with clinic staff to provide holistic care to patients, including referrals to other medical providers and community services.
• Will perform injections.
