Job summary
Careview Transitional Care of Weston is seeking a dedicated RN/LPN/LVN to join as Charge Nurse for the PM shift (2pm to 10pm), Part-Time. You will oversee patient care, lead nursing staff, and ensure high-quality care during stays, coordinating with professionals, administering medications, and supporting patients and families.

Qualifications
- Valid RN/LVN/LPN license in the state of practice.
- Experience in acute care settings preferred.
- Familiarity with EMR systems such as PointClick Care.
- Strong knowledge of medical terminology and coding.
- Excellent communication with patients and families.
Responsibilities
- Supervise nursing staff and coordinate patient care activities.
- Conduct comprehensive patient assessments and develop individualized care plans.
- Administer medications and treatments as prescribed, ensuring protocol compliance.
- Monitor vital signs and respond promptly to changes in patient conditions.
- Provide IV therapy and manage catheter care as needed.
- Maintain accurate medical documentation in compliance with HIPAA.
- Collaborate with interdisciplinary teams for effective communication.
- Assist in triage and emergencies, prioritizing care with critical thinking.
- Support patients with developmental disabilities, memory care, and geriatrics.

- Supervise nursing staff and coordinate patient care activities
- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed, ensuring compliance with established protocols
- Monitor vital signs and respond promptly to any changes in patient conditions
- Provide IV infusion therapy and manage catheter care as needed
- Maintain accurate medical documentation in compliance with HIPAA regulations
- Collaborate with interdisciplinary teams to ensure effective communication regarding patient needs
- Assist in triage and emergency situations, utilizing critical thinking skills to prioritize care
- Support patients with developmental disabilities, Alzheimer’s care, memory care, and other specialized needs
- Educate patients and families on health management, medication administration, and post‑discharge planning
- Valid RN/LVN/LPN license in the state of practice
- Experience in acute care settings such as hospitals or long‑term care facilities preferred
- Familiarity with EMR systems such as PointClick Care
- Strong knowledge of medical terminology, ICD coding (ICD‑9/ICD‑10), and patient assessment techniques
- Previous experience in critical care, emergency medicine, or geriatrics is highly desirable
- Ability to perform essential nursing skills including venipuncture, phlebotomy, medication administration, and wound care
- Excellent communication skills for effective interaction with patients, families, and healthcare teams
