Infinite Medical P.C. is hiring a Primary Care Nurse Practitioner/Physician's Assistant to provide on-site care to residents in long-term care settings. This part-time position offers meaningful patient relationships and a collaborative care environment.

The role requires an active MA license as an NP/PA/physician, 6 months LTC experience, and strong communication and clinical skills. On-site care, competitive compensation, and supportive teamwork are provided.

Qualifications

• Active and unrestricted NP/PA license in MA.
• Minimum 6 months of long-term care or skilled nursing facility experience.
• Strong communication, clinical, and critical thinking skills.
• Patient-centered elder care passion.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and goals.
• Ensure a safe, respectful, and supportive environment for residents.
