Carease Health - PA is seeking a compassionate Mental Health Provider to join our Collaborative Care team in skilled nursing facilities. You will provide emotional support, brief therapy, and mental health assessments to residents, helping them feel heard, supported, and emotionally connected within their environment.

This role offers regular supervision with psychiatric providers, thorough documentation, and collaboration to deliver comprehensive care.

Qualifications

• Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field
• 1 year of experience in mental health or related setting
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
• Strong communication, documentation, and relationship-building skills

Responsibilities

• Visit designated facilities according to schedule with supervisor
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
• Implement short-term, solution-focused therapy and supportive interventions
• Document visit notes and maintain detailed records after each visit
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP
