Job summary

SageLife is seeking an experienced Licensed Practical Nurse (LPN) to join Artisan at Hudson in Hudson, Massachusetts. The LPN will provide care, a safe environment, and emotional support to residents, while supervising Resident Care Assistants and ensuring care meets We Care values and Sage Associate Promise in alignment with state and corporate guidelines.

The position involves administration and documentation of medications, ongoing care planning, and collaboration with the Health & Wellness

Qualifications

• Associate’s or Bachelor’s Degree in Nursing.
• Minimum of three years’ experience in personal and/or long-term care with at least one year in a supervisory role.
• Knowledge and competency in medication administration; familiarity with personal care regulations and procedures.

Responsibilities

• Provide care, a safe environment, and emotional support to residents.
• Supervise Resident Care Assistants; provide education, evaluations, and counseling.
• Administer medications and treatments; ensure proper documentation and HIPAA compliance.
• Document all care in the EHR system and report concerns to Health & Wellness Director.
• Maintain annual training and perform other duties as assigned.

Artisan at Hudson is looking to hire an experienced LPN to join our amazing team!
POSITION SUMMARY: The LPN provides care, a safe environment, and emotional and psychological support to residents. The RN/LPN supervises Resident Care Assistants and ensures that all care demonstrates the We Care values and Sage Associate Promise and complies with state, federal, and corporate guidelines.

• *Responsible for care, medication and treatment delivery for residents.
• *Supervise the Resident Care Assistants, providing education, evaluations and counseling when needed, ensuring that they are delivering superior care to residents and demonstrating the We Care values and the Sage Associate Promise.
• *Responsible for the administration, storage and accountability of medication and treatments.
• *Ensure medications and treatments are administered and properly documented. Administer medications and treatments when required.
• Complete all documentation protocols for daily care, emergency care, monthly reports, charting, and medication management. Ensure that all documentation complies with HIPAA regulations.
• *Document all care in the Electronic Health Records (EHR) system.
• *Report resident concerns or changes to the Health & Wellness Director.
• *Ensure Resident Care Assistants are properly documenting care in the EHR system.
• *Keep current with state and facility required annual training.
• *Perform other duties as assigned.

QUALIFICATIONS:

Education: Associate’s or Bachelor’s Degree in Nursing.
Experience :Minimum of three years’ experience in personaland/or long term care with at least one year in a supervisory role.
Skills :Knowledge and competency in medicationadministration and familiarity with personal care regulations andprocedures.Highly committed to residentcare and have strong organizational, computer, interpersonal and communicationskills.
Licensures and Certifications: Registered Nurse or Licensed Practical Nurse inMassachusetts
