Qualifications
- Graduation from an LCME- or ADA-accredited medical school or equivalent with ECFMG if required.
- Board certification in Family Medicine or Internal Medicine as applicable.
- Active PA medical license and DEA registration.
Responsibilities
- Assess and treat patients according to established protocols and preventive care guidelines.
- Refer patients to specialists or social services as appropriate.
- Perform comprehensive physical exams and document findings.
- Educate patients and families about care plans, medications, and resources.
- Collaborate with the care team to coordinate transitions and goal alignment.
- Complete POLST and advance directive documents as appropriate.
- Involve ancillary services when needed and ensure compliance with standards.
