Job summary

Aveanna Healthcare in Falcon, NC is seeking a part-time Speech Therapist to deliver quality care per physician orders. Responsibilities include assessments, treatment, care planning, and coordinating with the multidisciplinary team in home-health settings.

Must hold a Master’s in Speech-Language Pathology, an active SLP license, and CPR certification; prior acute care experience preferred. You’ll evaluate needs, write plans, and communicate progress to physicians, families, and staff.

Qualifications

• Master’s degree in Speech-Language Pathology from an ASHA accredited program.
• Current and unrestricted SLP license to practice in the state.
• Current CPR certification and licensure requirements are satisfied.

Responsibilities

• Provide Speech Therapy services as prescribed by the physician.
• Follow policies and conform to patient care standards and state laws.
• Write an evaluation describing therapy needs, goals and outcomes with treatment plan.
• Develop a Plan of Care for Speech Therapy goals and outcomes.
• Maintain plan of care aligned with identified goals and outcomes.
• Coordinate services with health care team, families, and discharge planning.
• Maintain patient and company confidentiality.
• Communicate effectively with staff and families.
