Job summary

Interim HealthCare Hospice in Novi, MI is seeking a compassionate Hospice Registered Nurse (RN) for a part-time, home-based role. You will deliver individualized end-of-life care, manage symptoms, and support families within a six-month life expectancy framework.

Qualifications

• Graduate of an accredited nursing program.
• Current, unrestricted RN license in Michigan.
• Minimum of two years of nursing experience; hospice or home health preferred.
• Current CPR certification.
• Knowledge of state and federal home health and hospice regulations.
• Strong clinical assessment and nursing skills.
• Excellent communication and interpersonal skills.
• Compassionate, patient-centered approach.

Responsibilities

• Provide skilled nursing care to hospice patients in their homes, focusing on symptom management, comfort, and quality of life.
• Conduct comprehensive nursing assessments and monitor vital signs.
• Administer prescribed medications and treatments per care plan.
• Collaborate with an interdisciplinary team including physicians, LPNs, CNAs, and social workers.
• Coordinate medical equipment and supplies for patient care.
• Educate and support patients and families regarding disease progression and hospice philosophy.
• Provide emotional support to patients and loved ones throughout end-of-life journey.
• Maintain accurate clinical documentation and ensure regulatory compliance.
• Participate in rotating on-call coverage and travel as needed within service territory.
