UnitedHealth Group's Heart 'n Home Hospice, part of the Optum family, is hiring a Part-Time Hospice Nurse Practitioner to provide compassionate palliative care and symptom management. You will conduct face-to-face visits, coordinate plans of care, educate patients and families, and document care with the interdisciplinary team.

Required qualifications include an active nursing license, NP/CNS certification, and 5+ years of nursing experience.

Qualifications

• Active unrestricted nursing license in your state of residence.
• Current NP or Clinical Nurse Specialist certification.
• Certification by ANCC, NBCHPN, or AANP.
• Current CPR certification.
• 5+ years of nursing experience.
• Valid driver’s license and access to reliable transportation.

Responsibilities

• Conduct face-to-face visits with qualifying Hospice patients within specified time frames, sharing clinical findings with the Hospice Medical Director.
• Collaborate with the Hospice Medical Director and communicate recommended services to the primary physician, working alongside other healthcare providers as necessary.
• Initiate and coordinate comprehensive plans of care, documenting issues, goals, interventions, and patient/family responses to hospice care with the care team.
• Educate and guide patients and families on self-care techniques when appropriate.
• Maintain detailed and relevant clinical notes regarding patient conditions and any identified needs, reporting these insights to the Administrator and Hospice Medical Director.

Compensation for this role typically ranges from $104,500 to $156,000. Total cash compensation includes base salary and bonuses, influenced by factors including local labor markets, education, and experience, with potential increases based on productivity and performance.
