Job summary

Medi Home Health & Hospice seeks a Part-Time/32 Hospice Chaplain to provide direct spiritual support to patients and families in Culpeper, VA and surrounding areas (within a 50 mile radius). The chaplain collaborates with staff, clergy, and community groups to address spiritual concerns during terminal illness and bereavement care, and participates in IDT meetings.

The role requires at least one unit of CPE and a Master of Divinity from an accredited seminary.

Qualifications

• CPE required.
• Master of Divinity from an accredited seminary or school of theology.
• Hospice or hospital chaplain experience preferred.
• Experience with patients/families facing life-threatening illness and death.
• Hospice experience preferred.
• Valid driver’s license.

Responsibilities

• Provides direct spiritual support to patients and families.
• Works with staff, clergy, and community groups to address spiritual concerns of patients and families.
• Participates in patient conferences by assessing spiritual needs and reporting on services.
• Provides bereavement follow-up services as defined in procedure.
• Makes contact with clergy or appropriate representatives for patients/families.
• Develops and maintains a resource group of clergy for delegation of spiritual care tasks.
• Arranges for on-call availability of chaplain services.
• Visits patients and families in the home (minimal after-hours work).
• Attends biweekly IDT meetings.
