Job summary

MSA Home Health in Richland and Lexington Counties, SC, is seeking a part-time RN to provide skilled, patient-centered care in the patient’s home. You will perform initial assessments, develop care plans with physicians, and monitor progress to support independence and reduce unnecessary hospital visits.

The role requires active SC RN licensure, completion of an accredited nursing program, and at least 1 year of clinical nursing experience (home health preferred).

Qualifications

• Accredited nursing program completed.
• Active South Carolina RN license.
• At least 1 year of clinical nursing experience (home health preferred).

Responsibilities

• Provide one-on-one skilled nursing care to home health patients.
• Perform comprehensive initial assessments and develop individualized care plans with physicians.
• Monitor, evaluate, and document patient progress and adjust care plans as needed.
• Deliver education and support to patients and families to empower home care management.
• Ensure all care aligns with agency policies, clinical guidelines, and physician orders.
• Participate in rotating on-call responsibilities to ensure continuity of care.
