Qualifications

• Minimum of one (1) year of experience as a professional nurse, preferably in home care.
• Must have a valid driver's license and current CPR certification.
• Must pass a criminal background check.

Responsibilities

• Create and manage a plan of care for individual patients.
• Furnish services requiring substantial and specialized nursing skill.
• Evaluate the patient’s ADL and iADL abilities.
• Communicate patient updates effectively with MD and other staff.
• Participate in ongoing staff meetings and in-services.
