Job summary

Enhabit Home Health & Hospice is seeking a part-time home health professional to administer skilled care to patients in their homes. You will teach patients and families, and coordinate with the health care team under a physician’s plan of care.

The role requires a nursing license, CPR certification, and reliable transportation. The position covers 24 hours per week over four days and offers benefits aligned with a home health provider.

Qualifications

• Graduate of an approved school of clinical education.
• Licensed in the state where they currently practice.
• At least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
