Job summary

Beacon Home Health is seeking a Home Health Social Worker to assess patients’ psychosocial status, develop resource plans, and support in-home care planning. The role requires a Master’s in Social Work and valid WA licensure (MSW/LICSW), with at least one year of health care experience; BLS certification and a valid driver’s license are also expected.

Home health or hospice experience is preferred. You will collaborate with physicians and community agencies to ensure comprehensive care, quality

Qualifications

• MSW degree from an accredited program.
• Current/licensed MSW or LICSW in WA required.
• Minimum of one year experience in health care.
• Home health or hospice experience preferred.
• Maintains current BLS certification.
• Must have a valid driver’s license and insured vehicle.

Responsibilities

• Assess the psychosocial status of patients related to the patient’s illness.
• Develop and provide community and governmental resources to support the patient at home.
• Provide in-home social evaluations and plan interventions.
• Participate in the development of individualized care plans.
• Assist physicians in understanding social and emotional factors.
• Participate in quality assessment and performance improvement teams.
• Serve as liaison between patients and community agencies.
• Maintain and develop contracts with public and private agencies.

Education

Master’s degree in Social Work (MSW)

Licensed MSW or LICSW in Washington State

Home Health Social Worker responsibilities:

• Assess the psychosocial status of patients related to patient’s illness




• Develop and provide multiple community and governmental resources to support a patient in their home environment




• Provides assessment in patient’s residence and carries out social evaluations and plan interventions




• Participate in the development of the individualized plan of care




• Assist physician in understanding significant social and emotional factors




• Participate in quality assessment performance improvement team




• Serve as liaison between patients and community agencies




• Maintain and develop contracts with public and private agencies

Qualifications:

• A graduate of an accredited master’s program in social work




• Licensed MSW or LICSW in Washington State




• Minimum of one year’s experience in health care




• Experience with home health or hospice preferred




• Possesses and maintains current BLS Certification




• A licensed driver with an insured automobile
