Job summary

First Choice Home Health in Utah seeks an experienced RN Case Manager to provide skilled home health nursing on a part-time basis, with flexible scheduling. Primary care locations include Eagle Mountain and Saratoga Springs.

You’ll work with an interdisciplinary team, conduct assessments, develop care plans, educate patients and families, and coordinate with MDs and therapists. This role rewards compassion, teamwork, and strong organizational skills.

Qualifications

• Current Utah RN license is required.
• Reliable transportation is required.
• Excellent communication and interpersonal skills are essential.

Your Role as a RN:

We are seeking a Registered Nurse (RN) Case Manager to provide skilled nursing care to our home health patients. This position is available as part-time, with flexible scheduling. Care locations would be primarily in Eagle Mountain & Saratoga Springs.

You’ll work closely with an interdisciplinary team to:

• Conduct admission and OASIS assessments
• Perform head-to-toe and wound evaluations
• Create and implement individualized plans of care
• Provide follow-up skilled nursing visits

Pay and Benefits

• $50 to $105 per visit

Qualifications:

• Current Utah RN license
