Job summary

Choice Health At Home in Denver, CO is seeking a compassionate Home Health Aide (HHA) to support patients in their own homes part-time. You will deliver direct personal care, assist with daily living activities, and follow the care plan under RN supervision to promote comfort and independence.

The role emphasizes infection control, accurate documentation, and respectful communication with patients, families, and care teams. CRT guidelines and HomeCare HomeBase experience are a plus.

Qualifications

• Current CNA/HHA & LTC certifications.
• Valid Driver’s License.
• Valid Auto Insurance.
• CPR Certification.
• HomeCare HomeBase experience preferred.

Responsibilities

• Provide direct patient care to patients under direction of the RN and according to the Aide Plan of Care and to include personal hygiene, ambulation, exercise, safe transfers and other supportive tasks as assigned.
• Appropriately report changes in the patient’s condition.
• Complete appropriate documentation in a timely manner.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees.
• Provide a clean, safe and comfortable environment which promotes respect for patient, privacy and property.
• Provide skills necessary to assist the patient with proper nutrition and adequate fluid intake.
• Promote the agency philosophy and administrative policies to ensure quality of care.
