Job summary

Penobscot Community Health Care in Bangor is seeking a BC/BE physician (MD or DO) to join our Community Care and Geriatrics service as a part-time provider at a Skowhegan facility. The CCG serves aging adults in nursing homes and long-term care in Bangor and surrounding areas, including Belfast, Camden, and Skowhegan.

Qualifications

• BC/BE physician status in family medicine, internal medicine, or geriatrics.
• Interest or experience in geriatrics welcomed.
• Willing to work in Skowhegan and surrounding area facilities.

Responsibilities

• Complete skilled admissions for facilities.
• Respond to acute patient needs and coordinate care.
• Maintain strong relationships with facility staff and PCHC leadership.
• Facilitate effective communication and partnerships between PCHC and facilities.
