Total Care Therapy is seeking a compassionate Physical Therapist to join our London, Ohio team as a part-time clinician. You will assess residents, design individualized treatment plans, and deliver gait, balance, and mobility interventions to improve independence in aging populations.

Qualifications

• Current State Physical Therapist (PT) license.
• CPR/BLS certification.
• Valid driver’s license with reliable transportation.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the EMR.
• Educate residents, caregivers, and facility staff on treatment plans and home exercise programs.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.
