Job summary

Our Family Health in Florida is seeking a mission-driven Family Medicine or Primary Care Physician to join our outpatient team. The role focuses on preventive care, chronic disease management, and care coordination within a community health setting.

The position offers a flexible part-time schedule (2–3 clinic days per week) and collaborative support from a Medical Director and care team. Ideal candidates demonstrate strong communication and a commitment to evidence-based practice.

Qualifications

• MD or DO from an accredited medical school.
• Active, unrestricted Florida medical license in good standing.
• Unrestricted license to prescribe controlled substances.
• Training and experience sufficient to practice Family Medicine or General Practice.
• Board certification in Family Medicine preferred.

Responsibilities

• Provide comprehensive outpatient primary care services to adult patients.
• Evaluate, diagnose, and treat acute and chronic medical conditions.
• Perform physical examinations and order appropriate diagnostic testing.
• Develop and manage individualized, evidence-based treatment plans.
• Provide preventive care, health screenings, immunizations, and wellness counseling.
• Prescribe and monitor medications as clinically appropriate.
• Coordinate referrals to specialists and community resources when needed.
• Maintain accurate and timely documentation within the EHR.
• Collaborate with clinical, behavioral health, and administrative staff to support integrated patient care.
• Promote culturally responsive, equitable, and compassionate healthcare delivery.

Position Summary

The Family Medicine / Primary Care Physician will provide comprehensive outpatient primary care services for patients across the lifespan. This role focuses on preventive care, chronic disease management, acute care, and care coordination while supporting OFH's commitment to health equity and accessible healthcare.

The physician will work closely with the Medical Director and interdisciplinary care team to improve patient outcomes and strengthen community health.

Key Responsibilities

• Provide comprehensive outpatient primary care services to adult patients.
• Evaluate, diagnose, and treat acute and chronic medical conditions.
• Perform physical examinations and order appropriate diagnostic testing.
• Develop and manage individualized treatment plans utilizing evidence-based medicine.

Required Qualifications

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution.
• Active, unrestricted Florida medical license in good standing.
• Unrestricted license to prescribe controlled substance medications.
• Training and experience sufficient to practice Family Medicine or General Practice.
• Strong clinical judgment and commitment to evidence-based care.
• Excellent communication and interpersonal skills.
• Ability to work independently while collaborating effectively within a multidisciplinary team.
• Commitment to serving diverse and underserved patient populations.

Preferred Qualifications

• Board Certification or Board Eligibility in Family Medicine or a related primary care specialty.
• Experience in community health centers, FQHCs, safety-net healthcare organizations, or similar environments.
• Experience caring for medically underserved populations.
• Familiarity with electronic health records and quality improvement initiatives.
