Job summary

Laguna Rainbow Corporation seeks an attentive staff member to assist elders with transportation to and from the facility, seating, lift assistance, and meal deliveries. You will coordinate routes, maintain schedules, and ensure vehicle safety while upholding resident dignity and confidentiality.

The role requires reading and writing English, a valid driver's license, and ability to work under pressure in a care-focused environment within a nursing facility.

Qualifications

  • High school diploma or its equivalent.
  • Ability to read, write, speak, and understand English.
  • Must successfully complete state driver safety training.
  • Intermediate to advanced skill level in Microsoft Office or similar software.
  • Ability to use office equipment such as computers, phones, photocopiers, filing cabinets and fax/scan machines.
  • Ability to make independent decisions when circumstances warrant such action.
  • Excellent verbal and written communication skills.
  • Ability to organize and be efficient.
  • Must be knowledgeable of regulations governing activity services in nursing care facilities.
  • Ability to multi-task, work under pressure, and manage time/workload.
  • Must have strong social skills to interact with residents, families, and staff.
  • Must maintain a calm environment and warm atmosphere in the facility.
  • Must have a good driving record, valid driver's license, vehicle insurance and a reliable vehicle.

Responsibilities

  • Assist elders in and out of vehicle and to and from the main door of the facility.
  • Assist elders with seating and fastening seatbelts.
  • Assist elders with lift or ramp, securing tie downs, and stowing mobility equipment.
  • Report misconduct of passenger and/or drivers and notify supervisor as necessary.
  • Check daily transport schedule with front desk staff/transportation coordinator/social services.
  • Coordinate daily transportation routes and driver needs; maintain schedule.
  • Arrange for backup transportation as needed.
  • Confirm pick-up times or cancellations with elders.
  • Monitor vehicle maintenance needs and schedule service.
  • Keep vehicle interior and exterior clean at all times.
  • Prepare and follow up on accident/incident reports.
  • Report referrals to other transportation services.
  • Maintain accurate mileage records.
  • Make daily deliveries of hot and frozen meals to elders' homes; ensure temperature control.
  • Assemble home-delivered meals to maintain temperature and ease of transport.
  • Keep track of transportation units for monthly reports.

Position Functions:
  • Assist elders in and out of vehicle and to and from the main door of the Facility
  • Assist elders with seating and fastening seatbelts
  • Assist elders with lift or ramp, properly securing tie downs, and stowing mobility equipment securely in the vehicle
  • Report misconduct of passenger and/or drivers. Resolve complaints and notify supervisor as necessary.
  • Check daily transport schedule with front desk staff/transportation coordinator/social services.
  • Coordinate daily transportation routes, vehicle use, and number of drivers needed, communication with the front desk staff/transportation coordinator/social services department. Maintain schedule
  • Arrange for backup transportation as needed
  • Confirm pick-up time or cancelations with elders.
  • Monitor vehicle maintenance needs, schedule vehicle maintenance and pick up vehicle for service
  • Keep vehicle interior and exterior clean at all times
  • Prepare and follow up on all accident and incident reports
  • Report referrals to other transportation services
  • Maintain accurate mileage records
  • Make daily deliveries of hot and frozen meals to eider's homes and maintain quality of temperature control at all times. Meals will be delivered directly to the door of each elder.
  • Assemble home delivered meal to ensure temperature control and ease of transport to the home, supporting the cooks to get meals out within designated timeframe.
  • Keep Track of transportation units for monthly reports.
Personnel Functions:
  • Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facility.
  • Maintain confidentiality of all pertinent employee information.
  • Report known or suspected incidents of fraud to the Senior Center Manager.
  • Make certain that departmental computer workstations unattended are properly logged off or the password protected automatic screensaver activates within established facility policy guidelines.
Staff Development:
  • Attend and participate in appropriate in-service training programs as scheduled.
Equipment and Supply Functions:
  • Maintain and work within supply budget. Report unanticipated orders to Supervisor for approval prior placing orders.
  • Monitor that work/assignment areas are clean and equipment and supplies are properly stored before leaving such areas on breaks, end of day, etc.
Budgeting and Planning Functions:
  • Report suspected or known incidence of fraud relative to false billings, cost reports, kickbacks, etc.
Resident Rights:
  • Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.
  • Knock before entering a resident’s room.
  • Report all allegations of resident abuse and/or misappropriation of resident property lo Administrator immediately.
  • Perform other duties as deemed necessary and appropriate, or as may be directed by the Supervisor.

  • Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision, and the ability to adjust focus.
  • Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
  • Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
  • May be necessary to assist in the evacuation of residents during emergency situations.
  • Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as to reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
  • ls subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.
  • May be subject to the handling of and exposure to hazardous chemicals.
  • The noise level in the work environment is usually low to moderate.
