Job summary

Valley View Hospital’s Cardiopulmonary Rehab Department seeks a Registered Nurse Diabetes Educator to provide diabetes assessment, education, counseling, and self-management training for outpatients and inpatients. The role also supports cardiopulmonary rehab diabetes counseling, education, operations, and outreach.

The position is 24 hours per week, part time, with compensation guidance provided and a goal to improve patient outcomes through education and coordination of care.

Qualifications

• Certification or Licensure: Registered Nurse, Registered Dietitian or Pharmacist.
• CDCES or BC-ADM preferred
• Pump Trainer Certification or willingness to obtain
• Education:
• Experience: At least 1 year working with a diabetic patient population.
• Skills: Good computer skills; excellent customer service and communication skills.

Responsibilities

• Educates and counsels on diet, exercise, and chronic disease management.
• Assesses nutritional status and interprets lab values with documentation.
• Develops care plans for diabetes and obesity including inpatient and education programs.
• Supports DSME/DSMS accreditation trainings and education.
• Plans community events, health fairs, and presentations.
• Maintains knowledge of program core competencies.
