Job summary

The Community Care Cooperative (C3) seeks a Pharmacist II for part-time coverage at Harwich Port and Wellfleet Health. This role oversees medication distribution, collaborates with physicians, performs medication reconciliations, and supports pharmacy operations onsite.

You will supervise technician workflow, ensure compliance with state and federal regulations, maintain inventory including 340B, and provide patient counseling.

Qualifications

• Graduate degree from an ACPE-accredited pharmacy program.
• CPR or CPR/AED certification is required.
• MTM certification is preferred.
• Bachelor of Pharmacy or Doctor of Pharmacy degree is required.

Responsibilities

• Provides pharmacy information by answering questions and counseling patients on drug therapies.
• Develops clinic staff’s pharmacy knowledge by participating in clinical programs and training staff, students, interns, residents, and professionals.
• Performs medication reconciliation and comprehensive medication review based on organizational/patient needs.
• Identifies patient-specific barriers to adherence and develops plans to improve medication-taking behavior.
• Complies with state and federal drug laws and regulations.
• Maintains records for controlled substances and manages 340B inventory.
