Job summary

Live Well Homecare Agency, LLC is seeking a Registered Nurse to provide direct patient care in home settings, performing assessment, diagnosis, planning, implementation and evaluation within established nursing standards.

You will promote patient- and family-centered care, educate on illness management and post-treatment care, develop and document care plans with clear goals, collaborate with the health care team, patients and families, and maintain safety in a role that is safety-sensitive

Qualifications

• Nursing diploma or associate’s degree required; RN licensure required.
• Preferred: Bachelor’s Degree in Nursing.
• No supervisor experience required.
• Relevant clinical nursing experience may be considered.

Responsibilities

• Provide direct patient care including assessment, diagnosis, planning, implementation and evaluation within nursing standards.
• Promote patient- and family-centered care and educate on illness management and post-treatment care, including medication administration.
• Develop, implement and document individualized care plans with clear goals; collaborate with healthcare team, patients and families.
• Assess physical, psychosocial and emotional needs, respecting patient preferences, values and culture.
• Maintain safety; comply with regulations as this position is safety-sensitive.
