Job summary

Horizon Health in Bay Minette, AL seeks a Part-time Case Manager (Bachelor's in Social Work) for our behavioral health program at Mobile Infirmary and North Baldwin Infirmary. The Case Manager I will assess patients, guide discharge planning, and support families, working under a Social Worker II supervision.

Responsibilities include developing care plans, coordinating referrals, and participating in interdisciplinary teams, with occasional weekend sessions as needed.

Qualifications

• Bachelor's degree required
• Experience in case management/discharge planning required
• 0-1 years preferred
• Provisional State license in Social Work (LCSW-A) required
• Valid Driver’s License and/or ability to commute to work required
• Experience in hospital case management preferred
• Keeps up to date on professional literature
• Maintains competency in hospital policies and training

Responsibilities

• Implements a treatment plan of care for patients based on assessment under guidance of a LCSW
• Participates in referral development processes and activities as directed
• Engages in timely discharge planning with family, nursing staff and physician
• Participates in interdisciplinary and treatment teams; attends unit/hospital meetings
• Conducts individual, group and education psychotherapy sessions as directed and provides therapies as needed
• Provides resources and support for patients before, during, and after treatment
• Submits CQI+ outcome data weekly
• Performs other duties as assigned
