Imagine Pediatrics is seeking a Pediatric Home Care Specialist (Paramedic Certified) to join an interdisciplinary team providing episodic and longitudinal in-home care for children with complex medical needs.

You will travel to members’ homes to deliver high-quality care, coordinate with APPs, nurses, pharmacists, and social workers, and document in the EMR (Athena) while using Imagine’s app for family communication.

Qualifications

• EMT-Paramedic with active license in TX or NREMT
• PALS, ACLS, and BLS required
• 3+ years of field clinical experience; pediatric experience valued
• Valid driver’s license with good driving record and reliable transportation
• Comfort driving long distances in varied weather conditions
• Strong critical thinking, communication, and collaboration skills
• Experience using multiple technology platforms in care delivery

Imagine Pediatrics is a tech enabled, pediatrician led medical group reimagining care for children with special health care needs. We deliver 24/7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans to break down barriers to quality care. We do not replace existing care teams; we enhance them, providing an extra layer of support with compassion, creativity, and an unwavering commitment to children with medical complexity.

This role requires at least 80% travel to support episodic in-home care operations across various markets.

The schedule is expected to be Monday – Friday 10am - 6pm local time with flexibility based on member needs, visit locations, and virtual acute care support.

Applicants must reside in one of the following states: Florida, Tennessee, North Carolina, Missouri, Texas, Georgia, Louisiana, or District of Columbia.

What You’ll Do

As a Pediatric Home Care Specialist (Paramedic Certified) with Imagine Pediatrics, you will in collaboration with an interdisciplinary team, facilitate in home delivery of episodic and longitudinal care. You leverage an integrated technology platform and are complimented by an entire interdisciplinary team including APPs, registered nurses, navigators, pharmacists, psychiatrists, social workers, and therapists. You will:

• Travel to members’ homes, or wherever in the community our members are, in response to requests for acute and episodic medical, behavioral, and social needs.
• Work closely as a member of an integrated care team to provide high-quality, compassionate, and skilled care to our pediatric members and their families.
• Assist, in collaboration with other members of the integrated care team, with urgent needs for our members. Provide therapeutic intervention under the direct guidance of a physician.
• Assist in the engagement and onboarding of new members by helping conduct and facilitate clinical intake visits.
• Engage with members recently discharged from inpatient admission or recently seen in the Emergency Department to prevent readmission, and identify and solve root causes of readmission.
• Document care in the EMR (Athena) and communicate with families through Imagine Pediatrics’ proprietary application.
• Utilize virtual platforms and digital tools confidently to support care delivery.
• Perform other duties as assigned.

What You Bring & How You Qualify

First and foremost , you’re passionate and committed to reimagining pediatric health care and creating a world where every child with special health care needs gets the care and support they deserve. You will need:

• EMT-Paramedic with active license in the state of Texas, and/or Nationally Registered Paramedic (NREMT)
• PALS, ACLS, and BLS required
• 3+ years of field clinical experience. Nice to have: experience with pediatric patients and/or non-traditional Paramedic roles such as: critical care, primary care, tele-health, community paramedicine, mobile integrated healthcare, emergency room, or value-based care delivery.
• Valid driver’s license and good driving record, along with reliable transportation.
• Comfort driving long distances in varied weather conditions.
• Strong critical thinking skills, excellent communication skills, and effective collaboration.
• Experience and ability to use and navigate multiple technology platforms in care delivery.

The role offers a base hourly range of $29/hr - $36/hr in addition to annual bonus incentive, competitive company benefits package and eligibility to participate in an employee equity purchase program (as applicable).
