Job summary

Phoenix Home Care & Hospice in Springfield, MO is seeking a Nurse Navigator (LPN or RN) to provide Comprehensive Disease Management (CDM) and Palliative care in a full-time role. Working Monday through Friday, 8am–5pm, you will serve Springfield and surrounding areas.

The Nurse Navigator will lead goals-of-care conversations, educate families, coordinate with healthcare providers, and connect patients with home health services.

Qualifications

• Active MO or Compact RN/LPN license.
• Active driver’s license.
• Hospice or Home Health experience preferred.

Responsibilities

• Connecting with patients and establishing consistent face to face encounters
• Establishing rapport with patients and initiating goals of care conversations
• Educating on the benefits and goals of the various homecare services and making appropriate referrals to those services when patient desires
• Provide assistance with advance care planning and disease specific advance directives
• Navigate patients through various care planning tasks such as: disease progression and goals of care conversations, coordination with specialistsand connection to available community resources
• Completing accurate and professional documentation after each visit
• Building and maintaining strong partnerships with community healthcare partners
• Connecting and communicating at a high level with Phoenix colleagues to ensure continuity and collaboration of care.
