Job summary

MultiCare Health System in Yakima is seeking a Social Worker Specialist (MSW) to coordinate care across the continuum, perform psychosocial assessments and provide interventions in collaboration with physicians and care teams. Qualifications

• Master's degree in Social Work required.
• Licensed in WA as LASW/LICSW/LSWAA.
• WA driver's license, good driving record and private auto insurance.
• Minimum five (5) years of recent medical experience.
• Health industry knowledge preferred.
• Healthcare financial assistance knowledge preferred.
• Leadership experience a plus.

Responsibilities

• Coordinate outcome-based cross continuum care coordination and psychosocial interventions.
• Perform psychosocial assessments and collaborate with physicians and care teams.
• Support population health initiatives and care transitions across the continuum.
