Job Overview

Job Details. Job Location: Steward Center - Savannah, GA 31406. Job Summary: The Palliative Care Social Worker serves as the program’s primary liaison, connector, and system navigator across the full continuum of care. This role bridges all care locations to ensure patients and families experience seamless, compassionate, and coordinated care. The PC‑SW plays a pivotal role in driving program growth, improving timeliness and appropriateness of referrals, and supporting continuity of care from inpatient to community settings. By proactively identifying eligible patients, facilitating goals‑of‑care conversations, and managing psychosocial complexity, this role frees significant provider time—allowing MDs and NPs to expand visit capacity and increase census.

Qualifications
  • Bachelor’s degree in Social Work from an accredited program.
  • Minimum 2–3 years of experience in medical social work, palliative care, hospice, oncology, behavioral health, or other complex care settings.
  • Strong competency in psychosocial assessment, crisis intervention, navigation of community resources, and complex care coordination.
  • Prior experience in hospital systems, emergency departments, or multi‑site healthcare environments.
  • Master’s degree in related field preferred.
  • Knowledge of local community resources, long‑term care, home health, and hospice systems.
  • Bilingual skills (as applicable to population served).
Physical Demands and Work Environment

Position requires moderate to intense visual concentration, reading, using a computer and reviewing patient charts. This position has daily contact with patients, families, medical facilities, physicians, physician office staff, vendors/suppliers and HSI staff requiring excellent communications and problem solving skills and professional demeanor. This position carries a risk of exposure to infectious diseases. This position encounters stressful situations with patients and their families regarding end of life care and the Clinical Manager must be able to accurately, timely and concisely document the patient/family encounter using a computer.

Position Type and Expected Hours of Work

A full‑time, exempt and salaried position works a 40‑hour work week, to include after‑hours and weekend rotations, to manage the department, attend meetings or functions. The position is required to be on rotation for Administrator on‑call. A part‑time or PRN, non‑exempt and hourly position; work hours vary based on patient census, referrals, and plan‑of‑care needs.
