Qualifications

• Associate's degree in nursing is required.
• Active RN license through the Kentucky Board of Nursing required.
• Two years of acute care experience required.
• Must obtain CHPN within 3 years of hire.
• BSN preferred.
• One year hospice/palliative care experience preferred.

Responsibilities

• The Palliative Care Referral RN will work with the Palliative Care team to support clinical staff and physicians in the management of the continuum of care for palliative care patients.
• This includes assisting with patient evaluation, level of care determination, patient management, and goals of care conversations.
• The RN will assist with the education of staff and physicians, including completing advance directives and EMS DNR forms.
