Responsibilities

  • Provide consultative and ongoing palliative care services for patients with serious or life-limiting illnesses.
  • Assess and manage complex symptoms including pain, dyspnea, nausea, anxiety, and other distressing conditions.
  • Facilitate goals-of-care discussions, advance care planning, and code status conversations with patients and families.
  • Collaborate with primary teams, specialists, and interdisciplinary care teams to optimize patient outcomes.
  • Participate in inpatient consult services and outpatient clinic coverage, as applicable.
  • Provide education to patients, families, and clinical team members regarding palliative care principles.
  • Support transitions of care, including hospice referral when appropriate.
  • Maintain timely and accurate documentation in accordance with organizational standards.
  • The ideal candidate is committed to delivering high-quality, empathetic care while collaborating closely with physicians, advanced practice providers, nursing, social work, and other interdisciplinary team members to ensure a coordinated continuum of care.
