Job summary

AccentCare is seeking a part-time Nurse Practitioner for Palliative Care assigned to AdventHealth Central Texas Hospital in Texas. The role involves developing care plans, patient assessments, symptom management, and coordinating with physicians to ensure optimal patient outcomes.

The position requires NP licensure in Texas, DEA, CPR (preferred), and willingness to travel to multiple locations. Schedule is 20 hours weekly, Monday–Friday, 8:00am–5:00pm, with a $2,500 sign-on incentive.

Qualifications

• Graduated from an approved school of professional nursing.
• One year experience as a Nurse Practitioner is preferred.
• Licensed to practice as a Nurse Practitioner in the state of operation.
• Active DEA license in the state of practice.
• Possess and maintain CPR certification while employed in a clinical role (preferred).
• Must be a licensed driver able to travel to all business locations.

Responsibilities

• Develop and implement clinical plans of care for patients with chronic and terminal illnesses.
• Perform patient assessment, history and physical; provide symptom-management recommendations.
• Monitor patient progress and communicate with supervisory physicians and team members.
• Coordinate care with attending physicians and other care agencies to optimize patient outcomes.
• Prescribe medication as permitted by state regulations.
• Support hospice-appropriate continuum of care and education programs.

If you meet these qualifications, we would love to meet you:

Responsibilities:

As a Palliative Care Nurse Practitioner, you will:

• Develop and implement clinical plans of care for patients with chronic and terminal illnesses
• Perform patient assessment, history and physical; recommendations on aggressive symptom-management
• Monitor patient progress; update the Medical Director, Team Director and team members; consult regarding discharge plannning; and patient/family teaching
• Maintain communication with the supervisory physician (s) and attending physician to review patient records and care plans on a routine basis
• Initiate communication with attending physicians, other Palliative Care staff members and other agencies as needed to coordinate optimal care and use of resources for patient/family
• May provide support at time of death
• Prescribe medication as permitted by the state and/or the State Board of Nursing
• Participate in continum of care for patients that are hospice appropriate and ongoing education programs.
