Live Well Homecare seeks a Palliative Nurse Practitioner (Hospital-Based) to respond to inpatient referrals, perform comprehensive assessments, manage symptoms, and coordinate care with the interdisciplinary team in a hospital setting.

Qualifications

  • Master's or Doctoral degree in nursing with NP certification (Adult or Family).
  • Preferred hospice or palliative care nursing experience.
  • Knowledge of palliative care frameworks and effective communication.
  • Proficiency in computer use, problem-solving, critical thinking, and patient care planning.
  • Maintains continuing education, licensure, and CPR.
  • Valid driver’s license and willingness to travel, including evenings, weekends, and holidays.

Role:

Palliative Nurse Practitioner (Hospital-Based)

Responsibilities include:
  • Respond promptly to inpatient palliative care referrals and perform comprehensive assessments addressing pain, symptoms, psychosocial, emotional, spiritual distress, and functional status.
  • Develop and implement individualized care plans collaborating with an interdisciplinary team.
  • Prescribe medications and treatments within scope, conduct ongoing reassessments, and facilitate discussions on prognosis, treatment options, code status, and advance directives.
  • Support patients and families through complex decision-making and lead or support goals-of-care conversations.
  • Coordinate care transitions to outpatient or hospice services, and serve as a clinical resource for hospital staff regarding palliative and hospice eligibility.
  • Document assessments and care plans accurately in electronic medical records, ensuring compliance with policies and regulations.
  • Participate in quality assurance, utilization review, and performance improvement activities.
