The palliative care nurse practitioner (NP) is assigned to the Hosparus Health palliative care community and reports to the Senior Nurse Practitioner. The NP provides direct patient care, including making medically-necessary palliative care visits (billable) and hospice face-to-face recertification (FTF) visits (non-billable). The NP will also rotate to cover general Hosparus Health-wide weekday and weekend call for orders and prescriptions. In addition, the NP supports and coordinates care with the Interdisciplinary Group (IDG) and participates in education, outreach and leadership activities for the community location. The NP may be assigned additional patient care or administrative duties at the discretion of the Senior Nurse Practitioner or VP/Chief Medical Officer.
