Qualifications

• Graduate of accredited nursing program and NP program.
• Current APRN licensure in good standing.
• NP certification in Hospice or Palliative Care within 1 year.

Responsibilities

• Perform age-appropriate H&P for complex patients independently.
• Order and interpret diagnostic and therapeutic tests.
• Prescribe pharmacologic and non-pharmacologic treatments.
• Support chronic conditions to reduce hospitalizations.
• Assist with palliative care within regulations.
• Monitor intervention effectiveness and outcomes.
• Coordinate transitions across care settings and referrals.
• Educate patients and families on palliative care options.
• Document accurately in medical records with proper codes.
