Job summary

Hospice-Care-in-the-Berkshires is seeking a Palliative Care Program Coordinator to provide consultations and lead the ongoing development of the palliative care program. The role includes mentoring APPs, collaborating with leadership, and advancing quality of care through education and protocol development.

The position emphasizes education of staff and community partners, quality improvement, and involvement with local and national professional activities to advance hospice care.

Qualifications

  • Understands hospice and palliative care philosophy and serious illness conversations.

Responsibilities

Position Summary:

The Palliative Care Program Coordinator is a highly skilled professional with the primary responsibility of providing palliative care consultations within their scope of practice in addition to coordinating the growth and development of the palliative care program. The Palliative Care Program Coordinator has a key role as the program coordinator of all other HCIB and PVH APPs, by leading, educating, and mentoring Palliative Care Providers within the organizations. ---

Essential Job Functions
  • Palliative Care Program Coordinator collaborates with leadership regarding program goal planning, education, and program development and growth by maintaining up to date knowledge of regulatory and practice changes as they relate to the provision of palliative care.
  • Participate in planning as well as provision of the ongoing education of agency staff and community partners (local community college, local health system, etc.) through workshops, in-services, bed-side consultation, etc. Education includes, but is not limited to issues surrounding symptom management, medication management including concepts of goal-concordant prescribing, Serious Illness Conversation, etc.
  • Palliative Care Program Coordinator collaborates with the Palliative Care team on appropriate care interventions as needed.
  • Participate in the review and development of practice protocols by proposing interventions that are consistent with palliative care literature and research, including the development of quality assessment and performance improvement related to palliative care (ie MIPS tracking and submission).
  • Participate in program and professional development within the adult palliative care department, including, but not limited to the orientation and education of staff and stakeholders.
  • Participate in quality performance improvement projects to drive high-quality care in adult palliative care department.
  • Participate in the advancement of Hospice & Palliative care through involvement with local, state, and national trade organizations.
  • Maintains a regular panel of palliative care patients providing consultations and collaborating with primary and specialty care providers.
  • Responsible for contacting assigned patient and/or family to arrange a meeting to provide information to the prospective patient and/or family and/or referring professional.
  • Hospice Face-to-Face visits as needed with an assessment of the patient to be provided to the Medical Director to aid in determination of eligibility for hospice services will be completed and communicated to the patient and family.
  • Responsible for informing the primary providers and referral sources the outcome of the Nurse Practitioner consultations and evaluations via recommendation and collaboration.
  • Maintain regular contact with referral sources as needed.
  • Complete, maintain and submit accurate and relevant clinical notes regarding patient’s condition and care. Record pain and symptom management changes and/or outcomes as appropriate.
  • Point of Service documentation is an expectation of all clinical personnel for quality care, accuracy, and communication standards. As an Advanced Practice Provider, the clinician is expected to appropriately document their visit outcomes within 24 hours of their consultation.
  • Participate in the resolution of interpersonal conflict and issues of clinical and ethical concern as needed with palliative care patients, families and/or caregivers.
  • Participate in research activities or special projects as assigned and needed.
  • Other related duties as assigned.

Qualifications

Experience:
  • Minimum of five (5) years of professional experience in health care.
  • Experience in a hospice care required.
  • Experience in palliative care required, preferably 2+ years
  • Experience in homecare preferred.
  • Understands hospice and palliative care philosophy, and issues of death/dying, goal concordant prescribing and serious illness conversations and goals of care establishment.
Education and Training:

(Acquired through formal education, outside study, training on jobs of lesser degree, or by any combination of these. May be expressed in terms of formal educational equivalents):

  • A master’s or doctoral degree in nursing or physician assistant studies
License, Certification & Registration:
  • Possesses and maintains current CPR Certification.
  • Current Massachusetts licensure: Nurse Practitioner or Physician Assistant
  • Hospice and Palliative Care Nurse Certification required
Other Requirements:
  • Prefer experience in operational care delivery coordination
  • Demonstrates good verbal and written communication, and organization skills.
  • Strong observation skills and problem-solving skills.
  • Experience working with an Interdisciplinary Team preferred.
