Job summary

St. Elizabeth Physicians is seeking a Palliative Care APRN to join a diverse interdisciplinary team that includes physicians, chaplaincy, social work, RN, and pharmacy support.

The role covers inpatient palliative medicine consultations and outpatient palliative clinic responsibilities. The ideal candidate will have nurse practitioner experience in palliative/hospice care, excellent communication skills, and the ability to collaborate with physicians on symptom management and goals of care.

Qualifications

• NP license with hospice/palliative care specialization preferred.
• 1 year NP experience preferred; hospice or palliative care experience preferred.
• Strong observation, problem-solving, and nursing skills.

Responsibilities

• Reviews clinical information and completes new consultations and follow ups in a timely manner working with the interdisciplinary team (physicians, social workers, and chaplains)
• Determines goals of care for patients with cancer-related or life-limiting chronic illnesses; collaborates with palliative care physicians regarding symptom management needs.
• Provides extensive education regarding chronic diseases and how they impact quality of life.
• Communicates with treatment team including other providers on changes to plan of care.
• On call coverage required for palliative care
