Job summary

Commonwealth Pain & Spine seeks an Advanced Practice Provider (APP/APRN/PA) to join our Lexington clinic. You will identify patient needs, document findings, and develop comprehensive care plans with physicians.

Injections and ordered therapies are part of daily duties in a fast-paced, team-oriented setting. The role requires licensure in Indiana or eligibility, DEA registration preferred, and proficiency with EMR and standard office software.

Qualifications

• Experience with EMR systems and standard office software.
• Ability to manage stress and make sound clinical decisions.
• Punctual and dependable with professional demeanor.

Responsibilities

• Identify and assess patient health care needs and plan care.
• Record findings and develop prognosis with physician plan.
• Refer patients to physician or specialists when needed.
• Perform pain management injections per protocols.
• Prescribe/Order diagnostics, therapeutics, medications, DME as indicated.
• Collaborate with physicians and health professionals on care plans.
