Job summary

The Social Worker for the Patient Aligned Care Team (PACT) serves within Community Based Outpatient Clinics, delivering essential psychosocial support to Veterans. You will participate in an interdisciplinary team, assess needs, develop treatment plans, coordinate referrals, and manage crisis situations in a primary care setting.

Applicants must hold a master’s degree in social work from a CSWE-accredited program and be licensed or certified at the independent level.

Qualifications

• Master’s degree in Social Work from an CSWE-accredited program is required.
• Licensed or certified to independently practice social work at the master’s level.
• For GS-9, licensure may be waived temporarily per VA policy.

Responsibilities

• Serve as Social Worker for the Community Based Outpatient Clinics (CBOCs) within the PACT.
• Provide psychosocial assessments, counseling, and treatment planning.
• Coordinate referrals and services with an interdisciplinary team and community partners.
