Job summary

The U.S. Department of Veterans Affairs Davenport Clinic is seeking a Staff RN to join the Patient Aligned Care Team (PACT) in Outpatient Care. The PACT RN provides direct Veteran care and care coordination within the Primary Care Department.

You will deliver evidence-based nursing care, support timely access to services, and collaborate with the care team to optimize patient outcomes.

Qualifications

• English proficiency for direct patient care.
• Graduate from an approved nursing program with ACEN/CCNE accreditation.
• Current, active RN license in the United States.

Responsibilities

• Provide direct Veteran care and care coordination within the PACT/Primary Care setting.
• Collaborate with the care team to coordinate patient services and follow-up.
• Adhere to VA qualification standards and professional nursing guidelines.
