Job summary

The VA Sarpy County Clinic in Papillion, NE seeks a Registered Nurse Care Manager (PACT) to lead patient-centered care within a multidisciplinary team. You will manage an assigned patient panel, coordinate transitions, and develop individualized care plans using evidence-based guidelines.

Responsibilities include health education, coaching, and ensuring coordination with VA services and community resources.

Qualifications

• English language proficiency required to perform in clinical settings.
• Graduate of an accredited nursing program with RN licensure; state nursing license required.
• Foreign educated nurses must have degree equivalency validation where applicable.

Responsibilities

• Deliver care using nursing process and evidence-based practice within the PACT team.
• Coordinate with multidisciplinary team to manage an assigned patient panel.
• Educate and coach patients and families on self-management and wellness.
• Document all patient interactions and monitor outcomes for population health.
