Job summary

Neighborhood Healthcare’s PACE program seeks a Social Work Manager to lead the social work team, coordinate care plans, and ensure high-quality, person-centered care for seniors. This onsite role emphasizes multidisciplinary collaboration and program excellence within Riverside County.

Neighborhood Healthcare PACE is a managed medical plan built around surrounding participants with a team of physicians, nurses, social workers, therapists and care coordinators to help them maintain good health and a good quality of life. Our goal is to keep our seniors happy and healthy at home surrounded by their family and community.

As a private, non-profit 501(C) (3) community health organization, we serve over 500k medical, dental, and behavioral health visits from more than 100,000 people annually. With two PACE centers located in Riverside County, our PACE program is positioned to serve over 650 senior participants.

The Social Work Manager provides leadership, operational oversight, and strategic direction for social work services within the PACE program. This role is responsible for ensuring the delivery of high-quality, person-centered psychosocial care while maintaining compliance with state and federal regulations. The Social Work Manager plays a critical role in supporting interdisciplinary collaboration, optimizing workflows, and addressing system-level barriers that impact participant outcomes. This position partners closely with leadership, clinical teams, and community stakeholders to enhance service delivery, improve participant experience, and support program growth. This position is fully onsite. Schedule: Monday-Friday, 8am-5pm.

Responsibilities
  • Provides leadership, supervision, and support to Social Work staff, fostering a culture of accountability, collaboration, and continuous improvement
  • Offers ongoing coaching, mentorship, and performance feedback to support professional development and clinical excellence
  • Monitors staffing patterns, caseload distribution, and service coverage to ensure operational efficiency and appropriate workload balance
  • Supports workforce planning, onboarding, and role development to meet evolving program needs
  • Maintains competency in Interdisciplinary Team (IDT) processes and facilitates IDT meetings as directed, ensuring effective coordination of care and operational alignment.
  • Ensures standardization of social work operations within scope; identifies, reports, and addresses variances with rationale to maintain alignment with program expectations.
  • Oversee the completion and quality of psychosocial assessments, reassessments, and care plan coordination
  • Provides consultation and guidance on complex cases, including high-risk participants, behavioral health concerns, and crisis situations
  • Supports interdisciplinary collaboration to ensure participant needs are addressed holistically
  • Reinforces best practices in trauma-informed care, behavioral health, and culturally responsive service delivery
  • Monitors program trends, including high-risk cases, service gaps, and barriers to care, and escalates concerns as appropriate
  • Leads and supports process improvement initiatives to enhance efficiency, participant outcomes, and team workflows
  • Oversee documentation quality, audit readiness, and adherence to internal standards
  • Review productivity metrics, service utilization, and departmental performance trends to inform decision-making
  • Ensures departmental adherence to all applicable state and federal regulations, including CMS and DHCS requirements
  • Participates in audits, quality reviews, and regulatory processes, ensuring timely and accurate responses
  • Monitors compliance trends and implements corrective actions or process improvements as needed
  • Maintains up-to-date knowledge of regulatory changes and translates requirements into operational practice
  • Serves as a liaison between Social Work, leadership, and interdisciplinary teams to ensure alignment and effective communication
  • Builds and maintains relationships with community partners, including SNFs, ALFs, and other service providers
  • Facilitates care plan meetings, family meetings, and participant-related discussions as appropriate
  • Promotes clear, timely, and professional communication across all levels of the organization
  • Assesses training and development needs and supports ongoing staff education and skill-building initiatives
  • Contributes to program planning, growth initiatives, and service expansion efforts
  • Identifies and addresses systemic barriers impacting access to care and service utilization
  • Participates in organizational initiatives aligned with quality improvement and strategic priorities
  • Leads, coaches, and develops Social Work staff, including hiring, onboarding, performance management, and disciplinary actions in coordination with Human Resources
  • Provides direction to Social Work staff to ensure alignment with established workflows, performance expectations, and organizational priorities
  • Conducts regular team meetings to promote communication, accountability, and alignment
  • Evaluates staff performance and supports ongoing development through coaching, feedback, and competency-based assessments
  • Ensures appropriate staffing coverage and operational support across Social Work services
  • Promotes adherence to organizational mission, values, and service standards
  • Partners with the Center Director and Quality and Compliance to review, investigate, and resolve participant grievances, appeals, and service concerns, ensuring timely resolution and appropriate documentation
  • Provides coverage for staffing gaps, including LOA, PTO, and high-volume periods, by stepping in to support operations as needed
  • Supports grievance and appeals processes and ensures timely follow-up
  • Assists with psychosocial programming, participant groups, and education as appropriate
  • Maintains relationships with community partners and referral sources
  • Performs other duties as assigned
Qualifications
Education/Experience
  • Current Basic Life Support (BLS) certification through an American Heart Association (AHA)-approved source is required upon hire and must be maintained as a condition of employment.
  • AHA-approved courses include an in-person, hands-on skills check with a certified instructor using a mannequin to demonstrate CPR and emergency response techniques.
  • Online-only BLS courses without a live skills check do not meet this requirement.
  • In addition, current CPR and First Aid certification are required for all PACE employees in accordance with CMS and DHCS requirements. First Aid training will be provided at onboarding and must be renewed annually.
  • Three years of social work experience working directly with elderly and frail adults and families required; four years of experience preferred

  • Valid CA Driver’s License and proof of auto insurance required
  • California Associate Clinical Social Worker (ACSW) - Preferred (or ability to obtain)

Pay range: $104,200 - $156,200 annually, depending on experience.
