Job summary

West PACE in San Marcos, CA is seeking a Clinic Triage RN to coordinate and provide direct and indirect nursing care to participants at the center and at home, collaborating with the PACE Provider, clinic staff, and the IDT to manage ongoing care and support clinic operations.

The role requires a CA RN license, BSN preferred, BLS certification, and at least two years of nursing experience with frail or elderly populations.

Qualifications

• Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.

Responsibilities

• Conduct nursing triage for PACE participants by assessing symptoms and urgency.
• Provide timely nursing assessment and intervention, coordinating same-day appointments and follow-up care.
• Notify PACE Provider and IDT of significant changes in participant condition.
• Administer nursing care including wound care, medication administration, and other RN services.
• Coordinate care to prevent avoidable ER visits and hospitalizations when appropriate.
• May conduct home visits to assess participants and coordinate follow-up care.
• Maintain accurate documentation of triage assessments and care coordination in the EMR.

Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality, affordable health and support services that preserve and protect their dignity, quality of life, and independence. Position Summary

The Gary and Mary West PACE (GMWP) Clinic Triage RN is responsible for coordinating and providing direct and indirect nursing care to participants at the center as well as at participant's home, working in close coordination with the PACE Provider, clinic staff and other interdisciplinary team (IDT) members to manage ongoing care, complex clinical situations, and to support the operation of the GMWP primary care clinic. The Clinic Triage RN also, collaborates effectively with colleagues and stakeholders to promote the GMWP values, team culture and mission.

Essential Functions:

• Conduct telephone and in-person nursing triage for PACE participants by assessing symptoms, identifying changes in condition, determining urgency, and establishing appropriate next steps in collaboration with the PACE Provider and IDT.
• Provide timely nursing assessment and intervention for acute and changing participant needs, including coordination of same-day appointments, follow-up care, treatments, and other appropriate services.
• Utilize clinical judgment and established protocols to identify conditions requiring escalation and promptly notify the PACE Provider and appropriate IDT members of significant changes in participant condition.
• Provide nursing interventions within the RN scope of practice, including wound care, medication administration, ordered treatments, and other nursing services as directed by the PACE Provider.
• Coordinate and facilitate appropriate care to help prevent avoidable emergency department visits and hospitalizations when clinically appropriate, while ensuring timely escalation for conditions requiring emergency care.
• May conduct home visits when clinically indicated to assess participants, provide nursing interventions, and coordinate follow-up care.
• Monitor and follow up on participant symptoms, conditions, treatments, and care needs to ensure continuity and timely resolution of clinical concerns.
• Collaborate with PACE Providers and members of the Interdisciplinary Team (IDT) to develop, implement, and update participant care plans based on clinical findings, participant preferences, and identified needs.
• Communicate pertinent clinical information to participants, caregivers, Providers, and IDT members and promptly report significant changes in participant condition or medical status.
• Assist with care coordination and transitions between care settings, including hospitals, emergency departments, skilled nursing facilities, specialty providers, and the participant's home.
• Provide education to participants, families, and caregivers regarding disease processes, medications, self-care, symptom management, and prevention strategies.
• Provide unscheduled and same-day nursing services, including wound care and other treatments within the RN scope of practice.
• Maintain accurate, timely, and complete documentation of all triage assessments, interventions, communications, follow-up, and care coordination in the electronic medical record.
• Provide backup coverage for other GMWP nursing functions as needed.
• Protect participant privacy and maintain confidentiality of participant, employee, family, and organizational information.
• Follow GMWP policies, procedures, clinical protocols, PACE requirements, and applicable federal and California laws and regulations.
• Participate in Quality Improvement activities, staff meetings, training, education, and other organizational initiatives.
• Perform other duties as assigned.

Education and certification:

• Graduate of an accredited school of nursing; Bachelor of Science (BSN) preferred.
• Unencumbered California Registered Nurse (RN) License.
• Current Basic Life Support (BLS) / First Aid Certification required.
• Original valid driver's license with proof of auto insurance.
• Clean driving record.

Skills and experience:

• Minimum of two years nursing experience in a clinical setting with a frail or elderly population.
