Job summary

Rock Entertainment Group is seeking a Certified Nursing Assistant (CNA) to support PACE participants in the clinic and other settings. Under the supervision of the PACE Nursing Staff, you will deliver personal care and assist with activities of daily living in line with the plan of care.

Kentucky certification (SRNA) is required, CPR must be current, and a valid driver’s license with insurance proof is needed.

Qualifications

• SRNA certification required in Kentucky.
• High school diploma preferred.
• 1 year of home health aide or CNA experience preferred.
• Must maintain CPR certification.

Responsibilities

• Deliver personal care and related services to PACE participants.
• Collaborate with the Interdisciplinary Team to follow the plan of care.
• Assist participants in clinic, home, or facility settings as needed.
• Ensure safety and infection control standards are followed.

Position Summary:

Under direction of the PACE Nursing Staff, the Certified Nursing Assistant (CNA) is responsible for the delivery of personal care and related services to PACE participants in collaboration with the Interdisciplinary Team (IDT) and in accordance with the plan of care (POC). Assigned participants are primarily in the clinic/day center but may also be in other settings such as a home, long term care facility, assisted living facility or hospital.

Position Qualifications:

• High school diploma preferred.
• State Registered Nurse Aide (SRNA) certification required in the state of Kentucky.
• One year home health aide or nursing assistant experience preferred.
• Minimum 1 year of experience working with a frail or elderly population or must receive appropriate training upon hire.
• Use of technology-based tools, including electronic medical records, preferred.
• Must maintain CPR certification.
• Must have a valid driver’s license in accordance to policy with proof of automobile insurance required.
• Knowledge of safety and infection control requirements for healthcare facilities.
• Knowledge of medical equipment use and maintenance.
• Ability to communicate clearly and effectively.
