Angels of Care Pediatric Home Health in Phoenix, AZ is seeking registered nurses (RN) and licensed practical nurses (LPN) to join our team for full-time and part-time shifts, days, nights, and weekends. As a home health nurse, you will provide one-on-one patient care in the home, perform assessments, administer medications and treatments, coordinate care, and educate families to improve outcomes.

Angels of Care nurses provide direct, one-on-one medical care and make a difference in the lives of their patients and families. We are looking forRegistered Nurses (RN) & Licensed Practical Nurses (LPN)to work full-time and part-time shifts (days, nights and weekends available). Job duties include but are not limited to:

• Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
• Conduct on-going patient care and assessments.
• Administration of prescribed medication, treatments, and therapies.
• Coordination of care
• Educate family members on patient clinical care to enhance positive outcomes
• Preventative initiatives to protect quality of care for patient
