Ready 4 Work in Philadelphia is seeking a Life Skills Professional to support clients through direct interaction during shifts, assisting participants in developing life, recovery, coping and daily living skills.

You will facilitate and document groups, implement monthly activities, promote a recovery-focused program culture, and help establish and attain Recovery Support Plan goals with participants.

Formación

• Must have strong interpersonal skills and the ability to document accurately.
• Familiarity with Recovery Model concepts and group facilitation preferred.
• Basic computer and data entry skills.

Responsabilidades

• Intervene in crisis and document all critical incidents.
• Monitor and enter demographic, intake, and billing data into Avatar.
• Assess, plan, implement, monitor and evaluate daily living skills per the Recovery Model.
• Coordinate with other agencies to ensure coordinated services.
• Develop RSP goals with participants every 60 days and support daily progress.
• Report to Program Manager about maintenance needs and determine best learning styles.
• Other duties as assigned.
