Job summary

Qualifications

• Master's degree from a CSWE‑accredited college or university required.
• 1–3 years of post‑master's social work practice under supervision.
• Knowledge of coordination of care and discharge planning, crisis intervention and supportive counseling.

Responsibilities

• Conduct comprehensive biopsychosocial assessments and develop individualized care plans.
• Provide crisis intervention, supportive counseling, case consultation and care coordination.
• Coordinate with internal and external resources to maximize patient outcomes and ensure smooth transitions of care.
