Job summary

HCA Florida Westside Hospital is seeking a Clinical Pharmacist to join a multidisciplinary rounding team in Florida. You will be responsible for safe, evidence-based medication use and patient advocacy as part of daily rounds.

Qualifications

• Pharmacist license in good standing with State Board of Pharmacy required.
• B.S. or PharmD in Pharmacy required or equivalent.
• Graduate of accredited residency preferred.
• Two years hospital pharmacist experience preferred.

Responsibilities

• Prepare and dispense medication orders per physician requests.
• Verify CPOE orders for accuracy and safety.
• Interpret medication orders and document in patient records.
• Troubleshoot Centralized Order Entry orders.
• Prepare sterile, chemotherapy, and parenteral nutrition medications per guidelines.
• Evaluate Total Parenteral Nutrition for appropriateness.
• Handle controlled substances per policy and law.
• Ensure safe, evidence-based, cost-effective therapies.
• Monitor drug interactions, duplications, and lab results.
