Job summary

Mercy Care Atlanta is seeking an Outreach Case Manager to engage individuals experiencing homelessness and connect them with health and housing services. This role emphasizes compassionate, trauma-informed care and collaboration with community partners to ensure coordinated services.

Responsibilities include outreach at shelters, developing individualized plans, crisis intervention, and assisting with housing and benefits applications. A valid Georgia driver’s license is required.

Qualifications

  • Bachelor’s degree in Social Work, Psychology, Sociology, or related field, or 3+ years of relevant experience.
  • Experience with PEH, mental health, or substance use populations preferred.
  • Knowledge of local resources for vulnerable populations.
  • Ability to work independently and in a fast-paced team environment.
  • Strong communication, organizational, and interpersonal skills.
  • Compassionate, non-judgmental, culturally sensitive approach.
  • Ability to maintain confidentiality and follow ethical guidelines.
  • Proficient in Microsoft Office and case management software.
  • Valid Georgia driver’s license required.

This role is critical to our mission of providing whole-person care to Atlanta’s most vulnerable populations.

What You Will Do
